Thank you for registering your daughter for Modbury Stake's Young Women's Camp.
Please note that although your registration provides us with vital information, it does not fulfil the Church's requirement for a permission slip, and you will still need to provide one for her. Your Young Woman will not be able to attend Camp without one!
This can be done by returning to the LDS Conventions home page, clicking on "Register Now", then choosing "View your Registration". Enter the Registration Reference Number found in this email and the last name, then click Lookup Registration.
Once you do this, you will be able to view the permission slip.
Download it to your phone or computer (I recommend using Adobe) then fill out this interactive digital version.

If, for some reason, you are not able to do this, please print and fill it out. It will still need to be returned to the above email address.
